Attendance Time:

You have to reach at Hospital at your appointment time. Your friends/relatives can join with you if you want bring them at appointment time.

Required Documents For Outpatients For Appointment:

(1). Required Health Card.

(2). Required Passport/ID Card Copies.

(3). Required your Appointment card (for follow up appointments).

(4). Required your Referral letter (for your first appointment).

Required Documents For Follow Up Patient Appointment:

(1). Required Appointment Card.

(2). Required Health Card.

(3). If your are Non Qatari Patients you have to pay fee for appointment.

(4). Mention your contact information if possible.

 Important Note:

You have to bring your all medical documents and other appropriate documents in original if possible.
